Heather Leigh Reid Alberico PT, DPT, CSRS, C-IASTM, AIB-VRC

Heather Alberico earned her Doctor of Physical Therapy (DPT) from the University of St. Augustine. Prior to that, Heather obtained her Bachelor of Science in Applied Physiology and Kinesiology from the University of Florida.

Heather specializes in vestibular, stroke, and manual physical therapy and is most interested in treating BPPV, strokes, and total knee replacements.

In her free time, Heather enjoys spending time with her son, Kingsley, reading, drinking coffee, and traveling.

Jacky Americain CMPTA

Jacky Americain earned his Bachelor of Applied Science in Health Care Administration from the College of Central Florida. Prior to that, Jacky obtained his Physical Therapist Assistant (PTA) from the College of Central Florida.

Jacky is a Certified Manual Physical Therapist Assistant (CMPTA) certified by FPTA, Clinical Instructor (CI) credited by APTA, and is most interested in treating ortho patients, especially the knees and shoulders, patient education on safety and body mechanics, and sports injuries.

In his free time, Jacky enjoys spending time with his family, going on trips, and family vacations. He also enjoys officiating soccer games, playing soccer, going to soccer tournaments, going to the gym, running, watching reality TV shows and sports, and mentoring new students.

Nicholas Arcadipane

Harley Barnes PTA

Harley Barnes earned her Associate of Science in the Physical Therapist Assistant program from College of Central Florida.

Harley is most interested in treating orthopedics.

In her free time, Harley enjoys being outdoors, reading, and spending time with her family, friends, and dog.

Pam Bulloch PTA

Pam Bulloch earned her Associate of Science in the Physical Therapist Assistant (PTA) program from the University of Alabama, Birmingham.

She is most interested in treating orthopedics.

In her free time, Pam enjoys gardening and dancing.

Jillian Burns PTA

Jillian Burns earned her Associate of Applied Science degree as a Physical Therapist Assistant from the College of Central Florida in 2023. She also holds an Associate of Arts degree from Wake Technical College and previously obtained her certified nursing assistant certification from Withlacoochee Technical Community College.

With a strong foundation in patient care, Jillian enjoys working in orthopedic, geriatric, and sports physical therapy, and is dedicated to helping each patient reach their highest level of function and well-being.

Cheyenne Fackler PTA

Cheyenne Fackler earned her Associate of Science in the Physical Therapist Assistant program from Keiser University.

Cheyenne is most interested in treating post-op orthopedic.

In her free time, Cheyenne enjoys hanging out with her boys and camping.

Anthony Failla PTA, CCI, C-IASTM, Specialty Certification in Orthopaetics

Anthony Failla earned his Associate of Science at the College of Central Florida PTA program. He has been in health care for over 19 years now.

Anthony has earned his “Specialty Certification in Orthopaetics” through EIM and is a “Credentialed Clinical Instructor” and also holds certifications in “IASTM”. He is most interested in treating orthopedic conditions, pre and post-surgical as well as geriatric conditions.

Colby Frowick PTA

Colby Frowick earned his Physical Therapist Assistant from Florida Gateway College.

Colby is most interested in treating low back pain and balance.

In his free time, Colby enjoys hunting and fishing, exercising, and Gator football games.

Jamey Gage PTA

Jamey Gage earned his Associate of Science as a Physical Therapist Assistant from Broward College. Prior to that, Jamey earned his Bachelor of Science in Physical Education from Saint Leo University.

Jamey is most interested in treating shoulder pathology in the throwing athlete.

In his free time, Jamey enjoys fishing, golfing, and traveling.

Madelyn Harper PTA

Madelyn Harper earned her Physical Therapist Assistant from the College of Central Florida. Prior to that, she earned her Bachelor of Science in Health Science from University of South Florida.

Madelyn enjoys helping patients achieve their goals and improve their quality of life
through physical therapy. She has a passion for pediatrics and sports injuries.

In her free time, she likes to travel, play with her dog, and stay active.

Lee Jacobson PT

Lee Jacobson earned his Master of Science in Physical Therapy from Loma Linda University. Prior to that, Lee obtained his Bachelor of Science from Loma Linda University.

Lee specializes in outpatient generalist/advanced training in manual therapy and is most interested in treating orthopedics.

In his free time, Lee enjoys four-wheeling in his Jeep, fishing, and RVing.

Richard Luevano OT, CHT, COMT-UE

Occupational Therapist for 40 years and Certified Hand Therapist since 2010. Certified Orthopedic Manual Therapist-Upper Extremity since 2015. Experience in working with traumatic hand, wrist and elbow injuries, burns, repetitive motion injuries, amputations, fractures, tendonitis and nerve compression disorders. Skilled at custom orthotic/splint fabrication. Enjoy helping people regain function and improve their quality of life after suffering injuries. Outside of work enjoy spending time with family and watercolor painting.

Deborah Main

Kelly Pearce Baez PT, DPT

Specialities: Neuro, Orthopedics, Pediatrics, Pelvic, Vestibular

Dr. Kelly Pearce Baez, PT, DPT holds a Doctorate in Physical Therapy from Nova Southeastern University and a Bachelor of Science in Health Sciences from the University of South Florida. Dr. Baez worked as a Doctor of Physical Therapy at BayCare, AdventHealth, and Tampa Bay Physical Therapy caring for patients with varying health conditions that included neurologic, pediatric, pelvic, orthopedic and vestibular disorders.

She is a dedicated mother, wife, pet owner, and lover of nature. With a background in teaching, Dr. Baez believes that health education and the development of oneself are key pillars to achieving quality of life.

Dr. Baez is Bilingual (Spanish and English) and helped raise money with her friend for a pro-bono physical therapy trip run by NSU Tampa to Puerto Rico following Hurricane Maria. Dr. Baez believes in giving back to the community to support all individuals and their families.

Anthony Lee Persampieri PT, DPT, MTC, IDN-Cert, AIB-VR.

Tony attained his Doctorate of Physical Therapy in 2018 from the University of St. Augustine. He has become certified in Manual therapy, Dry Needling and Vestibular rehabilitation with over 14,000 hours treating back pain, joint replacements, vertigo and sports injuries. He loves helping people get back to an active lifestyle either on the golf course, pickle ball, playing with their kids/grandkids or just walking around safely.

In his down time he enjoys playing sports with his kids, working out with his wife and spending time with his family on their farm.
